Summary 

The National Survey of Quality of Life and Health was first executed in 2000. This survey series was created to provide data that could guide the government's health objectives, plans, and policies in the following decade.

The 2000 survey gathered information through household, family, and individual questionnaires. The sample was designed to be nationally representative and 6228 individuals 15 and older participated. Those surveyed answered questions about health status, chronic diseases and pain, injuries, disability, risk behaviors, satisfaction with life, use of public and private services, and living conditions and environment.
